We respect your personal privacy and are committed to adhering to federal and state privacy laws and industry guidelines in order to protect you and your identity. This Privacy Policy describes how we handle the personal information that you provide to us through HealthGoal360.

Please be sure to read this Privacy Policy before using HealthGoal360 or submitting information to us. By using HealthGoal360, you accept and agree to our privacy practices. Our Privacy Policy may change from time to time and your continued use of HealthGoal360 means that you accept such changes, so please check this page periodically for updates to this Privacy Policy.

In addition, to reading this Privacy Policy, we recommend that you read our User Agreement and Code of Conduct as they provide other useful and important information regarding your use of HealthGoal360.

INTRODUCTION

HealthGoal360 is a personal health platform that lets you gather, edit, add to, store, and share health information about you and your family online. HealthGoal360 also allows you to share your health information with family, friends, health care professionals, mobile phone applications, health-related devices, and online tools.

INFORMATION THAT IS COLLECTED FROM YOU

Active Information Collection

When you visit HealthGoal360, we will not collect any personally identifiable or other sensitive information from you unless you provide it to us, or permit others to provide it to us, voluntarily. If you choose to register, during the registration process you will be asked for certain information, most of which is required in order to complete the registration. Some of the information that you submit during registration or other online processes, may be personally identifiable (that is, information that can be uniquely identified with you, such as your name, address, phone number, fax number, e-mail address, social security number, date of birth, etc.) and may include other sensitive information. Well inform you which information is optional. You can modify, add, or delete any optional account information by signing into HealthGoal360 and editing your account profile.

We also may post market research studies, either from third parties or our own. You participating in market research may reveal important information about you. Your participation in the market research studies is voluntary and we are not responsible for the privacy of the information you provide in those studies.

In addition, you may allow providers, program or systems to provide information to us about you. Such information will be handled in the same manner as all information that you provide to us directly.

We do not have control over the privacy policies applied by any other party to whom you may disclose information. Third party sites or programs that are linked to from the HealthGoal360 are not under our control and we are not responsible for their content or privacy policies. If you would like information on any other party’s privacy policy, you should contact that party directly, and we encourage you to do so.

Other than personal information that you voluntarily provide to us (e.g., through registration or market survey), we do not use or access individually identifiable information that you, or health care providers that you authorize, enter into your person record, unless we are specifically required to by law, in which case we will notify you, to the extent permitted by law. If you allow others to access your personal record, we cannot control the way that such third parties use, access or disclose your information.

Passive Information Collection

As you navigate through a website, certain information can be passively collected (that is, gathered without you actively providing the information) using various technologies and means, such as navigational data collection. Like most web site operators, we track some of this anonymous information. We passively collect information to understand what areas of HealthGoal360 are most popular, which areas may require improvement and what technologies are being used by our web site visitors. This information helps us to update and improve HealthGoal360. We also use this information for security purposes, to detect and to block security breaches and to provide you with a safe online environment. We do not collect personally identifiable information automatically, but we may tie information collected automatically to your previously submitted personally identifiable information.

HOW WE USE YOUR INFORMATION

We use your personal information to enhance the operation of HealthGoal360 and allow you to use its features. This may include using your information to identify which services might be useful to you or to provide you with important notices. We may aggregate your information with other users to identify trends and developed new services. We may also provide aggregate statistics about our visitors, online traffic patterns and related information to reputable third parties, but these statistics will not include any personally identifying information.

We will not sell your personally identifiable information to any outside organization without your consent. We may disclose your personal information to reputable vendors that assist us in processing information or delivering materials or services. These vendors are instructed not to use the information for any other purpose. At times, the information may be disclosed when legally required to do so, at the request of governmental authorities conducting an investigation, to verify or enforce compliance with the policies governing HealthGoal360 and applicable laws or to protect against misuse or unauthorized use of HealthGoal360. We reserve the right to transfer this information to a successor entity in connection with a corporate merger, consolidation, sale of assets or other corporate change concerning HealthGoal360.

We occasionally hire other companies to provide limited services on our behalf, such as answering questions about products and services or creating advertise displays. We give those companies only the personal information they need to deliver the service, such as IP address or e-mail address. We require the companies to maintain the confidentiality of the information and prohibits them from using the information for any other purpose.

We do not otherwise distribute to third parties your personally identifiable information. We, however, will have no liability for disclosure of information obtained due to errors in transmission or the unauthorized intervention of third parties.

HOW YOU CAN USE YOUR INFORMATION

By default, you are the custodian of any records you create. You may invite additional people to have access to your account and in some instance, they modify the information in your account. Some of the information stored in the records you manage may be highly sensitive, so you need to consider carefully with whom you choose to share the information. It is solely your decision as to whom you grant access to your account and HealthGoal360 has no responsibility or liability for the individuals to whom you grant access.

You can also share personal information and health information with programs and other systems. You decide which programs and systems you want to use. You must approve (or deny) the program's or system’s access. A user who has the appropriate level of access must affirmatively authorize a program's or system’s access to any health record in your account. We require such program or system providers to agree to provide accurate information about their privacy practices and comply with applicable laws. However, except for restricting the access they have to your HealthGoal360 data, we do not control or monitor the practices of those programs or services, and their privacy practices will vary. The access you grant a program or system through HealthGoal360 is valid until you revoke that access.

It is solely your decision as to which individuals, programs and systems you grant access to your account and HealthGoal360 has no responsibility or liability for the individuals, programs and systems to which you grant access.

PROVIDER ACCESS TO INFORMATION

If you identify your providers, we will allow your providers access to your information and we will provide them with aggregated reports about all of their patients that are part of HealthGoal360.

In addition, in the U.S., we enable participating providers to obtain reports about whether the information they send to a record is used. This feature supports the U.S. "meaningful use" objective of the HITECH Act, which provides incentives for health care providers to send their patients copies of their medical information electronically. Providers that participate can get reports that include:

  • A number that the provider uses to identify the patient within the provider's system
  • An indication if the user took one of the "qualifying actions" in HealthGoal360 during the time period of the report (but no information about what action)

“Qualifying action" currently includes activities such as viewing, downloading, or transmitting health information via email. You can turn off reporting for your records. You can learn more about meaningful use at http://www.healthit.gov.

CHOICES YOU HAVE ABOUT USES OF YOUR INFORMATION

You may contact us as noted below if you do not want your information to be used in the manner described in this Privacy Policy or wish to correct any information that you have provided to us. If you request a correction of your information, we will use reasonable efforts to make those corrections in our files.

With respect to cookies: We and our ad servers, sponsors and service providers and their advertising service providers use cookies to, as applicable, store visitors’ preferences, record session information, such as user preferences, record user-specific information on what pages users access or visit, customize Web page content based on visitors’ browser type or other information that the visitor sends. If you do not wish us to collect cookies, you may set your browser to refuse cookies, or to alert you when cookies are being sent. If you do so, please note that some parts of this site may then be inaccessible or not function properly.

Please note that we do not have access to, or control the information collected by, cookies or similar electronic tools used by third-party advertisers on HealthGoal360 or their advertising servers. The use and collection of your information by these third-party advertisers and advertising servers is not covered by this Privacy Policy. For information about advertising networks and how to “opt out” of cookies placed by these advertisers and their advertising servers, please contact the third party companies.

WEB BEACONS

HealthGoal360 pages may contain electronic images known as Web beacons (also referred t as Web bug, pixel tag or clear GIF) may be used to

  • assist in delivering cookies on our sites,
  • enable us to count users who have visited those pages, or
  • deliver co-branded services.

We may also include Web beacons in promotional e-mail messages or other communications in order to determine whether you opened or acted upon those messages.

We may also employ Web beacons from third parties to help us compile aggregated statistics and determine the effectiveness of our promotional campaigns. We prohibit third parties from using Web beacons on our sites to collect or access your personal information. We may collect information about your visit to HealthGoal360, including the pages you view, the links you click, videos you watch and other actions taken in connection with HealthGoal360. We also collect certain standard, non-personally identifiable information that your browser sends to every Web site you visit, such as your IP address, browser type and language, access times, and referring Web site addresses.

COMMUNITY AREAS AND SOCIAL MEDIA

We may provide an opportunity for you to communicate in community areas, postings, wikis, bulletin boards, chat rooms, blogs and other public and interactive forums on or through HealthGoal360 (“Community Forums”). Our Community Forums are open to the public and should not be considered private, and therefore, any information you share in any Community Forum (including Personal Information) is by design open to the public. We are not responsible for any information that you share in any Community Forum.

We may use third party administrators, agents or representatives to assist us in managing such Community Forums.

If you mistakenly post information on any Community Forum and would like it removed, please contact us to request its removal. We will try our best to accommodate such removal request – however, not all information may be removable.

MINORS

We are committed to protecting the privacy of children. We do not collect personal information from any person we actually know is under the age of 13. Although not recommended, a parent or guardian, however, may use HealthGoal360 for the benefit of a minor. The parent or guardian assumes full responsibility for ensuring that any information shared is kept secure and that the information submitted is accurate.

HOW YOUR INFORMATION IS PROTECTED

We have implemented both technical and procedural methods to maintain the integrity and security of all sensitive information under our control and protect it against loss, misuse and unauthorized alteration. In addition, our electronic files are backed up for redundancy and designed to be accessible only by authorized individuals.

This site contains links to other web sites operated by third parties that are provided as a courtesy to you. We cannot control these third party sites, which may collect personal information from you. When you follow a link and leave this site, you do so at your own risk.

CLOSING YOUR ACCOUNT/DELETING RECORDS

You can close your HealthGoal360 account at any time by editing your account profile. We wait 90 days before permanently deleting your account information in order to help avoid accidental or malicious removal of your health information. When you close your account, HealthGoal360 deletes all records for which you are the sole custodian.

You can delete any health record that you are a custodian of by editing a record's profile. If other users had any level of access to that record, the record no longer appears in their accounts. We wait 90 days before permanently deleting the record information in order to help avoid accidental or malicious removal of your health information.
